Abstract
As online betting and sports gaming expand rapidly across the United States, rates of gambling-related harm have risen sharply. Self-exclusion (SE) programs—voluntary tools that restrict a user’s access to gambling platforms—are widely promoted as essential harm-reduction strategies. Yet significant questions remain about their real-world effectiveness, especially within the fragmented U.S. regulatory landscape where state and operator enforcement varies widely.
This study investigates SE effectiveness using a mixed‑methods, cross‑sectional design. A Qualtrics survey of 832 adults (21+) with gambling experience and at least one SE enrollment captures behavioral patterns, motivations, enforcement experiences, barriers, and perceived outcomes. Quantitative analyses include descriptive statistics, chi-square tests, t‑tests, ANOVA, logistic regression, and supervised machine‑learning models (decision tree and SMOTE‑enhanced random forest). Qualitative, open-ended responses were analyzed using basic natural‑language frequency methods.
Findings show a 26.7% breach rate, with demographic variables showing no significant association with relapses. Instead, program‑level and experiential factors—such as ease of enrollment, access barriers, support availability, exclusion duration, and perceived exclusion efficiency—predict adherence. Individuals using government‑administered, centralized SE systems were significantly less likely to breach than those using operator‑specific or third‑party tools, highlighting the importance of cross‑operator coordination. Machine‑learning models (92% accuracy) identify “chasing losses,” high gambling frequency, behavioral distress, and low perceived tool efficiency as strongest predictors of breach.
Results show SE programs reduce gambling harm when implemented with streamlined enrollment, multi‑operator coverage, behavioral monitoring, and on‑going support services. The study recommends strengthening SE programs nationally by expanding centralized exclusion (e.g., NVSEP), improving accessibility, integrating counseling or peer support, and embedding behavioral‑analytics tools. As online betting and sports gaming continue to proliferate, these findings offer actionable guidance for regulators, policymakers, and gambling‑harm‑reduction stakeholders seeking to build accountable, evidence‑based SE systems.
